Collection of Patient Personal Information

At Enchantment Family Dentistry, we obtain a patient’s social security number (SSN) and make a copy of driver's license and insurance cards for several important reasons:

1. Identity Verification: Copying the driver's license helps us verify the identity of patients. This is crucial for ensuring that we provide services to the correct individual and maintain accurate patient records.

2. Insurance Processing: Having a copy of the insurance card allows us to efficiently process claims with your insurance provider. This ensures that we apply the correct benefits and assist in minimizing any potential billing issues.

3. Insurance companies use SSN as a patient identifier.  Without a patient’s SSN, it is much more difficult for our office to communicate with an insurance company on a patient’s behalf.  

4. Accurate Record Keeping: Keeping copies of these documents helps us maintain up-to-date and comprehensive records. This is important for compliance with privacy regulations and ensures we have the necessary information to support your dental care needs.

5. Emergency Contact Information: A driver's license often provides critical contact information that can be useful in the event of an emergency, helping us communicate effectively with emergency contacts or healthcare providers if needed.

Treatment Planning Overview

Should a cavity or other oral health condition requiring treatment be diagnosed, we will show the area of concern to the patient directly in the mouth or using intra-oral photos and x-ray images. Our office utilizes the computer monitor to display x-rays and photographs, ensuring you see everything as we do.

All treatment recommendations will be thoroughly discussed, and we encourage you to ask any questions you may have. We are always available for additional discussions if needed, whether by phone or in person.

 

Financial Overview

At Enchantment Family Dentistry, we strive for transparency in financial matters. We will inform you of the cost of treatment before any services are rendered to ensure there are no unexpected surprises. Payment is collected upfront to streamline the process and allow you to focus on the quality care you receive from Dr. Frizzell.

As a courtesy, we aim to work closely with your insurance company to maximize your benefits and minimize out-of-pocket costs. While our team is dedicated to assisting with insurance claims and providing estimates for covered services, it is important to understand that patients are ultimately responsible for knowing the specifics of their insurance coverage.

We encourage all our patients to verify their benefits and understand their policy details, including deductibles, co-pays, and any limitations or exclusions. If you have any questions or need guidance on how to obtain this information, our knowledgeable staff is here to help. Your proactive involvement ensures a smooth and transparent billing experience.

Radiography (X-ray) Overview

Our practice adheres to strict radiographic guidelines to ensure the safety and health of our patients. We utilize advanced technology which includes digital radiography to minimize radiation exposure. X-rays are only taken when necessary for Dr. Frizzell to make an accurate diagnosis. Rest assured, your family's health is our priority.

If you are not comfortable with taking dental x-rays under any circumstance, we are not a good choice for your dental care.

Alternative Medicine Overview

While we respect diverse beliefs regarding medical treatments, Enchantment Family Dentistry follows evidence-based practices. If traditional dentistry does not align with your preferences, we understand and are happy to discuss options or recommend alternative providers that may better suit your needs.

Scheduling Overview

To help you keep track of appointments, we offer appointment cards, email reminders, text alerts, and phone call reminders. You can choose your preferred method of notification, and all these services are provided as a courtesy to ensure your busy schedule is managed effectively.

We ask for 48-hour notice for cancellations to respect everyone’s time and to accommodate other patients. Should unforeseen circumstances arise, we assess each cancellation on a case-by-case basis.

At Enchantment Family Dentistry, we value informed decision-making. If you seek a second opinion, we're happy to provide one, ensuring our findings are independent of external influences. Conversely, if you'd like to seek further opinions, we're pleased to assist with transferring records to your chosen provider.
